Sunderland Weight Management Programme

 

 

From 17 August 2021, Sunderland will have a new, free 12-week weight management programme which is funded by Public Health England.

Participants on the programme will receive support tailored to their specific needs, work with our friendly wellbeing experts and meet like-minded people, supporting them to make positive and long-lasting changes to their health.

To qualify for the programme, participants must meet all of the following criteria:

  • • Be a Sunderland resident, or be registered with a Sunderland GP if they live outside the local authority boundary
  • • Be aged 18+
  • • Have a qualifying Body Mass Index of 30+ (27.5 for BAME). Please note the programme is not suitable for pregnant women.
  • Once registered for the programme, participants will begin with a one-on-one consultation which will be repeated at the end of the 12-week course, with a follow-up session at 26 weeks. The programme is a mixture of group support, education around healthy eating, sleep, exercise and wellbeing as well as tailored one-to-one sessions.

Residents can sign up directly, or ask their GP, or a healthcare professional such as a practice nurse to refer them by completing this form.

Training Practice

We would like to announce that from August 2021 Forge Medical is now a Training Practice.

Patient's will be able to book telephone consultations with the registrars.

Covid Vaccinations

The surgery will contact patients when vaccines are available for the group they belong to according to Government guidlines. We are unable to consider any requests which are not in the groups being vaccinated.

Appointments

Patient's please note: Routine/Urgent appointments are released at 08:00 a.m. every morning. These are available online  (if you are registered for Patient Access) as well as contacting the surgery by telephone. You may find that an e-consult is helpful, which is accessed through the website and a response is given at least by the end of the next working day.  Other alternatives may be offered such as an appointment at the Extended Access which are usually out of surgery hours.

WE ASK THAT IF YOU ARE UNABLE TO ATTEND YOUR APPOINTMENT TO PLEASE LET US KNOW AS SOON AS POSSIBLE - THIS WILL ENABLE SOMEONE ELSE TO BE SEEN IN YOUR PLACE.
